The transition to community-based care has been discussed for several years now. The aim is for primary care to be the hub of care and to interact with other health services, with the aim of providing patients with good, close and coordinated care.
The reform is large and comprehensive and should lead to increased accessibility, more involved patients and increased continuity. In practice, however, several obstacles have been identified for the operations to actually implement the transition and contribute to visible effects. Both IVO and the Swedish Agency for Health and Care Services Analysis have found that the right resources and conditions are lacking "on the ground".
To support organizations in implementing the transition, Sirona is actively working on several projects, on behalf of a number of different regions. The projects aim to contribute to person-centered care that is carried out close to the individual through working methods that make it easier for healthcare professionals.
Sirona has, among other things, implemented concrete and tangible operational projects to improve accessibility through, for example, better analysis of care queues to identify where bottlenecks occur. We also help develop structural measures, such as how to profile hospitals to focus care, which has proven to be effective. In addition, at Sirona we have developed tools that create new opportunities; for example, we have developed a digital twin of a healthcare system. This allows us to model the effects and benefits that arise when specific action initiatives are implemented. In many assignments, we have also worked on the digitization of care, which is a key factor in the transition to close care.
The transition requires a wide range of interventions, such as the introduction of digiphysical care, person-centered interventions and collaboration between services, while focusing on proactive and health-promoting care.
