Background information

One of Sweden's university hospitals is facing a major change journey where a new reimbursement model will be introduced from January 1, 2022. The new model is a tool that facilitates the hospital management's way of managing, planning and following up the hospital's operations. The introduction of the new reimbursement model will be a major change, moving from an appropriation-based model to a combination of new reimbursement principles. The decision to introduce a new reimbursement model is based on the hospital management's desire to reward productivity and quality.

Implementation

Sirona was tasked with supporting the program management in the change process and thus supporting the program and hospital management in several strategic choices. As part of the program management support, Sirona has helped to structure the work, create clear goals and work plans, while maintaining agile processes and methods. Sirona has also carried out dynamic modeling of all areas of activity and a large number of workshops, and all work has been done in close co-creation with the program leader and five sub-project leaders. One subproject has, among other things, worked on mapping how budget and control processes are linked today in order to then be able to develop a proposal for how this should work from 2022.

Outcome

Sirona has participated in several workshops with all area management teams and met with area and operational managers where, for example, the risks, weaknesses and strengths of the new remuneration model were examined. The assignment also included an economic analysis of the effects of introducing the new remuneration model. Sirona has taken responsibility for providing coherent change management, including both strategic and operational issues for the whole program, with all its sub-projects and work streams.

In the tender, the client set high requirements for experience in program and change management and work in the public sector, as well as that the supplier has a deep understanding of health care. Sirona meets all the requirements and has extensive experience of working with reimbursement models in various contexts; for example, investigation of the consequences of reimbursement systems for innovation, on behalf of the government's Life Science coordinator, analysis of reimbursement systems in primary care and elderly care in Sweden and has also worked with "quality-based financing" (goal-related reimbursement for quality) for all hospitals in Norway.
