Background information
Since its inception, Sirona has had a strong focus on developing innovative, proactive and predictive solutions to healthcare challenges. For a few months, it explored the possibilities of developing a model for better management of acute readmissions after hospitalization.
Challenge
Based on this work, a collaboration with researchers at KTH was initiated, and an application for funding from Vinnova was designed. The work was financed through the Vinnova call "Forska & Väx".
Implementation
Sirona carried out a thorough literature review to identify best practice in the field. Continuous dialogues were held with several county councils to map and understand their needs and conditions. Furthermore, a predictive model was developed, tested and validated for Swedish conditions. Further development and adjustment of the model took place in close collaboration with KTH.
Outcome
The model developed performed in line with the best international models and the work has led to a large number of productive dialogues with various county councils. The model has subsequently been further developed by Sirona to incorporate the latest methods in machine learning and predictive analytics.
